      CYNTHIA K. URBAN, 36, died Oct. 25 at her home in Covina.  Born in Fontana, she lived in Fontana 36 years before she moved to Covina eight months ago.  She was a caregiver.  She was a former parishioner of St. Nicholas Byzantine Church in Fontana.  Ms. Urban is survived by a daughter, Amber of Fontana; two brothers, Robert of Yucaipa and Peter of Georgia; and two sisters, Joyce Markos of Redlands and Christine Ulrich of Mission Viejo.  Services will be at 3 p.m. Thursday at Green Acres Memorial Park in Bloomington.  Ingold Chapel Funeral Home in Fontana is handling arrangements.
